As a charge nurse at Hawaii Kai Assisted Living, you will be expected to be able to complete the below:

0600: Shift starts

Shift report from noc shift
Count narcotics and sign count verification sheet
Check if any residents are on Alert Charting
Gather supplies: Black Radio (channel 4), RA radio, Pager, Keys to med cart, BP cuff, Thermometer (if needed)
Weights are taken on the ist and 2nd monthly, and weekly on Monday's... If it is a day for weights, try to obtain them BEFORE breakfast and record in the Weight Log Binder


Tasks to complete throughout the day

Medication Passes
Follow up with faxes- call MD and refax. Obtain Telephone orders from MDor NP when necessary
Document in communication log if necessary
Nursing/ Functional/Fall Risk Assessments
Communicate with RA's
Complete any new orders
Check call book
Document in 24-hour communication book
Medication check back from Pharmerica
Label medications correctly when received by Pharmerica, family or mail order
Refill medications in cart as needed
Refill medications if missed by NOC

CLEAN Nursing Station: wipe down counters, desks, phones, copier, med carts, take out trash, sweep/swiffer floor
Prepare shift report for oncoming shift

Be ready to give report by 1800!!


Be sure to update 24-hour log for new changes AND pass in report
Document in communication book for aides to read if it pertains to resident care


When a new/change/DC order is received:
-Put change on MAR
-Put on P.S.
-Fax Pharmerica for update
-Direction changes sticker on medication bottle or blister pack
-Pass along in report & document in 24-hour log
-Put copy of MD order in DON's box
-Remove D/C medications from cart and destroy
-Destroy narcotics with RN

Check to see if any monthly assessment need to be completed- functional, nursing and fall risk
Check to see if annual physicals and TB clearance for residents need appointments/scheduling with
Check call book and call families to bring in any meds/supplies needed

Medication check back:

Make sure label is correct

Write times on bottle cap/blister pack left corner

If current med supply, write checkmark B

Highlight out of call book if family brought in or from mail order


Chart completed by DON
CN: Nursing assessment & Fall Risk assessments
Compare medications brought in by family against MD

Clarify all orders-via fax or call MD

If resident is out for more than 7 days- needs to be assessed by RN

Upon returning to community- CN to redo nursing assessment and Fall Risk assessment

CN: Recheck all orders

Obtain Vitals, determine if resident needs to be sent out {If resident hit head, or when in doubt call 911- specify Assisted Living- and which building
If sending out; gather resident's POLST, MAR, Insurance info, face sheet, MD order form
Alert front desk that ambulance has been called
Update family, and administrator/DON
Fill out Incident Report
Document in resident's chart, on care plan, communication book for aides, 24-hour book, shift report, and alert charting form
Remove pendant from resident and save in office in an envelope
