Minimum Data Set (MDS) Coordinator – Ohana Pacific Management Co. Kauai

Ohana Pacific Management Co. Kauai is looking for genuinely caring Minimum Data Set (MDS) Coordinator who strives to provide clinical excellence and genuine care for our communities on Kauai. Our Minimum Data Set (MDS) Coordinator is responsible to ensure timely completion of accurate MDS assessments, care plans, and electronic MDS transmissions for our residents. Our Minimum Data Set (MDS) Coordinator also coordinate the Interdisciplinary Team in the development of a Resident Assessment Instrument process and reviews, edits, and ensures accurate MDS, Care Plans, and care conference notes.

Successful Minimum Data Set (MDS) Coordinator Applicants Must:
• Pass criminal background check, TB, and drug screen.
• Be eligible to be a licensed Registered Nurse in the State of Hawaii.
• Have a minimum of 1-year of MDS experience.
• (Applicants who do not meet ALL of the above requirements will NOT be considered.)
Hours:
Full-time: day, evening, and weekend shifts are available.
